How to include a customer satisfaction feedback after a form is submitted?

We have developed a form recently for our Business scenario and we would like to add in a feature. I am not sure if it is existing as of now in Smartsheets.
We need to send the user who submitted the form to fill an Satisfaction from scale of 1-5 along with smiles.
May I know how can this be done?

As long as the submitter either adds their email when submitting or if the form is set to that, they have to be logged in to access the form, we can collect their email and then use that with a workflow that triggers an update request or an alert with another form.

Okay, This has to be done in a new form and adding to this if I have to add the symbols in the form like Smiles version (- Good ;️- Neutral ;- Bad).
Is it possible to add something like this in the form, I have check few samples but none of them look like this.
